Please complete this form to submit the identity documents required for your application. This step is requested after your initial enquiry has been reviewed by our team.

お申込み内容を確認後、ご案内する本人確認手続きです。必要書類をこちらのフォームよりご提出ください。

This form is submitted securely online. Documents are uploaded directly to our secure, access-restricted storage system and accessed only by authorized MPMS team members for KYC/AML purposes, in accordance with our Privacy Policy. Each file may be up to 10MB.

本フォームはオンラインで安全に送信されます。ご提出いただいた書類は、アクセスが制限された安全な保管システムに直接アップロードされ、KYC/AML(本人確認・マネーロンダリング防止)目的でMPMSの権限あるスタッフのみが確認いたします(プライバシーポリシーに準拠)。1ファイルあたり最大10MBまでご提出いただけます。

Good: flat, all 4 corners visible, well lit, no glare 良い例: 平らに置き、四隅がすべて写り、明るく反射がない状態

Avoid: cropped corners, blur, or glare/reflection 避けたい例: 四隅が切れている、ぼやけている、反射・グレアがある

Passport Cover (front & back, opened flat in one photo) パスポート表紙(表紙を開いて表裏が一緒に写った1枚の写真)

Good: passport opened flat — plain back cover on one side, front cover (with country name/emblem) on the other, both fully visible in one photo 良い例: パスポートを開いて平らに置き、片側に無地の裏表紙、もう片側に国名・エンブレムのある表紙が、両方とも1枚の写真に写っている状態

Avoid: only one cover captured — please include both front and back in the same photo 避けたい例: 片方の表紙しか写っていない。表紙と裏表紙の両方を1枚の写真に写してください

Proof of Address (utility bill or bank statement, within last 3 months) 住所証明書類(公共料金請求書または銀行明細、直近3ヵ月以内)

Good: full page visible, your name & address clearly readable 良い例: ページ全体が写り、氏名・住所がはっきり読める

Avoid: cropped edges, blur, or glare/reflection 避けたい例: 端が切れている、ぼやけている、反射・グレアがある

Photo / Selfie (holding your passport, for anti-fraud verification) 顔写真/セルフィー(なりすまし防止のため、パスポートを持った状態)

Good: face clearly visible, passport photo page held open beside your face, plain background 良い例: 顔がはっきり見え、パスポートの顔写真ページを開いて顔の横に添え、背景はシンプルに

Bank Statement (last 6 months) 銀行口座明細書(直近6ヵ月分) Each page must bear the bank's official stamp/seal to be accepted. 各ページに銀行印(銀行の証明印)が押されているものをご提出ください。 If your statement is a multi-page PDF, please compress it first if possible. 複数ページのPDFの場合は、可能であれば事前に圧縮してからアップロードしてください。
